A picure of a "one hoss shay" was used by the Quality and Reliability
Engineers during the Apollo program at MSC, Houston, TX. It suggested that
the space craft needed to be designed with complete appreciation for
failure modes in all sub-systems such that no portion would threaten the
mission in safety or objective ..
